Getting started

 

Welcome to the new PennGen Laboratories Sample Submission website!  This website can be used to manage all DNA, metabolic, and hematologic test requests from PennGen Laboratories.

 

Before you begin, please maximize your browser window.

 

If this is your first time using the new website, you must create a new account.

Fill out the New Account form and be sure to indicate whether you are part of a veterinary office or if you are an owner/agent.

 

Note that your password must be at least 8 characters long, contain at least 1 uppercase letter (A, B, C, …) and 1 lowercase letter (a, b, c, …). 

 

After you have created your account, you will be asked to log in using the username and password you just created.

(1) Creating an animal

 

To request a test on your animal(s), you must first “Create an Animal” from the home screen:

 

 

 

Animal Official Name – If the animal you wish to test has an official name used at shows and for breeding purposes that would you like to appear on your results report, please provide that here.  Otherwise, you may just enter the animal’s regular name (e.g., Boomer)

   

Call Name – The name that you call the animal (may be the same as Animal Official Name)   

 

Species – Select Canine for dogs, Feline for cats

 

Breed – You must choose a breed in order for the system to provide you with the correct selection of testing options   

 

Birthdate – If you do not know the exact date of birth of your animal, please provide your best estimate

 

Gender    

 

Neutered

 

AKC – If animal is registered with organization other than AKC, please indicate here

  

Registration Number – May be AKC number or other organization as indicated above

 

Tom/Sire Name – if available  

 

Tom/Sire Registration Number – if available      

 

Queen/Dam Name – if available      

 

Queen/Dam Name Registration Number – if available      

 

Veterinarian Name – Please provide the name of the primary veterinarian you see for this animal (should be associated with the veterinary office you identified when creating your account)

 

Comments – Use this space to indicate any extra information regarding your animal.  You do not need to provide any clinical information at this point; this can be included with your test request, later.

 

 

When are you done filling out this form, you may either complete the animal record or you may create more from the same litter.  This option will decrease the amount of data entry you need to do by copying over common information such as breed, birth date, and parentage.

 

 

 

(2) Creating a submission

 

A submission may consist of one or more test requests (e.g., pyruvate kinase deficiency, metabolic screen, and Fanconi syndrome screen).  You may have one or more sample types associated with each test request (e.g., EDTA whole blood, urine, cheek swabs, and citrated blood).  In order to have your animal tested, you must create a submission to summarize all of these options.

 

Once you have created one or more animals, you will be automatically taken to a page entitled “Request a New Test.”  If you not wish to request a test at this time, simply click “Cancel” at the bottom of the screen and you will return to your homepage.

 

When you are ready to create a submission for an animal, select the animal you would like to test from the right hand side of the owner/agent home screen, and click on the “Create a Submission” button at the bottom of the screen.

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

(3) Creating a test request

 

To add one or more test requests to your submission, begin filling out the test request screen (the first screen you see after you click on “create a submission”).

 

Many of our tests are available for all breeds, but some are specific to certain breeds.  The list of available tests will depend on the breed you selected when you created the animal to be tested.

 

After you select the test, additional fields will appear to be filled in, including reasons for testing and sample types to be submitted.  Please note the required samples applicable to each test type. 

 

Once you have completed this form, click on “Create Test Request” and you will be taken to a screen from which you can decide what you would like to do next:

  • Add another test request to this submission. 
  • Mark the submission as Complete and proceed to the sending instructions. 
  • Go to the submission details without completing this submission. 
  • Go to the animal owner/agent details without completing this submission.  

 

(4) Completing a submission

 

Once you have added all necessary tests to your submission, you will have the opportunity to mark the submission as complete.  At that point, you will be taken to the sending instructions page.  You should print that page and send it in the package along with your sample(s).  You may wish to cut out the address box within the dotted lines and affix that to your package in order to ensure safe and timely arrival at our labs.

 

After you have printed the submission sheet, please click “Continue” to return to the owner/agent homepage.

 

(5) Checking submission status

 

Once you have submitted your samples, you can check on their status anytime by logging back in to the PennGen Labs website.  

 

 

There are generally four possible status options:

-          Waiting to be Received – your electronic submission has been accepted by the PennGen system, but we have not yet received your samples in the lab.

-          Received – your samples have been received but processing has not yet begun on the samples

-          Billedtesting has been completed but we are awaiting payment to make the results available so please make a payment online using your credit card

-          Completed – testing has been completed.  Your results have been sent to you via e-mail and are also available in .pdf format on the PennGen Labs website.

 

You may only delete your submission while it is still in “Waiting to be Received” status.  Once it has been received, you will need to call us to make special arrangements if you have changes to your submission.

 

 

 

 

 

(1) Creating an animal owner/agent record
